Medix is hiring a Community Health Worker in Oxnard, CA 93036!
Duties may include but are not limited to the following:
Conduct intake assessments for an average of 300 monthly referrals, verifying medical eligibility and coordinating transitions to Community Health Worker (CHW) services.
Assist clients with Medi-Cal eligibility and enrollment, ensuring financial and residential requirements are met before case activation.
Gather client demographic information, review nurse case notes, and schedule family interviews to initiate program services.
Facilitate communication with families, issue necessary authorizations, and manage documentation, including in-person, online, and mailed interactions.
Qualifications:
Required to be fluent bilingual (Must be able to read and write fluently)
Prior case management or CHW experience
Schedule/Shift:
Monday - Friday: 8am - 5pm
